The cause of death for Forest Whitaker’s ex-wife Keisha Nash has been revealed.

Nash passed away in December and her death certificate states that she died from “alcoholic liver disease.”

According to the document obtained by TMZ, she suffered from alcoholic liver disease “for years,” while “acute renal failure” — a symptom of anorexia — was “another significant factor” in her death.

Nash and Whitaker shared two daughters, True and Sonnet. Nash also had a daughter, Autumn, from a previous relationship.

Following her death, a source told DailyMail.com that Nash had been in and out of hospital in the months before she died with intestinal problems in the wake of her years-long battle with anorexia.

“Keisha battled anorexia for years. Forest stayed by her side during those difficult times, but the following year he filed for divorce in 2015,” the insider added.

“They remained friends and their divorce was amicable, and because Forest said his wife was, ‘kind, funny bright and outgoing’ — as well as a good mother to their children — there was no need to not remain friendly,” the DailyMail.com’s source added about their split.

Whitaker’s ex-wife sparked worries in 2014 after she was seen looking extremely thin at the Film Independent Spirit Awards with her husband. RadarOnline.com reported she could have lost around 35 pounds over the last few years.

As well as her work as a model and actress, Nash was an entrepreneur with a children’s clothing line and a cosmetics line called Kissable Couture.

Whitaker and she met in 1994 on the set of the film Blown Away.

“I had seen him in  A Rage in Harlem. Right away, I thought Forest had a gentle soul and a nice spirit,” she told Ebony about her then-husband.
